Office Policies
Thank you for choosing Davis Dermatology for your skin care needs. Our mission is to provide excellent healthcare to our community. Our staff is dedicated to making your experience exceptional. In order to better serve you, we ask that you adhere to the following office policies.
Appointment Policy
New patients, please arrive 15 minutes early to complete the necessary forms. These forms may also be completed online by clicking here.
Patients arriving more than 15 minutes late may need to be rescheduled or worked into the schedule if our calendar permits. Patients worked into the schedule may experience a longer than usual wait time. We cannot guarantee same day appointment if scheduled appointment is missed.
Please bring the following items to all office visits:
- Photo ID card
- Payment (in form of Cash, Visa, MC, AMEX, and Discover)
- Guardian (if 18 years or younger)
- Failure to bring the above items may result in rescheduling of appointment and may incur a cancelation fee of $35
- Bring your Military ID card if you’d like to claim the military discount
Email – Questions in regards to current treatment plan may be addressed. New or ongoing medical issues that require changing of medication or treatment plan, and other issues not previously discussed, require an office visit.
Prescription Policy
- Most prescriptions are written with refills.
- Please contact your pharmacy first to determine if you have any remaining refills.
- Follow-up evaluations are required to obtain prescription refills beyond the initial prescription and its refills.
- Please schedule follow-up appointments early so as not to run out of your medications.
- For patient safety, follow-up appointments are required for additional refills: every 3 months for prescriptions taken by mouth (orals), every 6 months for prescriptions applied to the skin (topicals). We apologize for any inconvenience as no exceptions to this rule will be granted.
- Cosmetic appointments not related to medical conditions are not considered “follow-up” appointments for medical conditions.
- Cosmetic patients requesting prescriptions for medical conditions must be checked in as a medical patient and will be assessed a medical visit fee of $175 which cannot be applied to cosmetic treatments.
- Isotretinoin (Accutane) requires monthly in-office follow-up appointments for refills.
Payment Policy
Payment is required at time of treatment, during the appointment
Medical visits: We do not accept insurance. Payment is required at time of each appointment. Acceptable forms of payment include: cash, Visa, MC, AMEX, and Discover.
Cosmetic procedures: Payment in-full is expected at time of service. Deposit are required for most treatments
Quotes for cosmetic procedures are valid for 30 days. Fees are not negotiable
